There seems to be a lot of anger and violence in the dream having to do with her. It doesn't necessarily mean that either of you is angry at the other or wants to hurt the other in real life, just that you are under some type of stress in waking life that is showing up in your dream.

Because the feet are really smelly, maybe it could mean that something "stinks" in your life, which would go with the idea of tension. A baby (your nephew) makes me think of something new, or something that is going to change for the better. So the idea of your nephew being crushed makes me think of something like hopes being shattered. This doesn't mean bad luck in real life. The dream isn't telling the future; it's just revealing feelings that you have that you may not have been aware of consciously. It could just be a signal to you to think about what's bothering you in your life and what you would like to change. This goes with Maljonic's idea that feet could symbolize moving forward in your life. I don't know if there is anything sexual about this dream. That's something you would have to think about yourself.
